Introduction

Welcome to the Case Study Portal. Diabetes Connekt aims to provide opportunities for deliberate practice. Case study review is a form of deliberate practice and offers an opportunity to mentor and ensure consistency in how new and experienced diabetes educators talk about and manage diabetes. ADEA members can review case studies submitted by peers or submit a case study for an Expert Advisory Group review. The case study portal is here to help you develop as a diabetes educator. No questions or challenges you are seeking assistance with are ‘silly’.  

Clinicians who submit a case study will receive a response from a moderator/s about their case. The response will be evidence-based, focused on creating consistency in practice, and provided in the form of prompts linked to evidence. The moderators will not provide specific clinical advice but will provide you with suggestions, direction and resources to assist you with your case and to develop your assessment skills. Ultimately the clinical actions you take following the response is your decision.

Instructions for submitting a case study

  1. Check out the Case Study Repository and see if a similar case to yours is already loaded that might help you with your challenges. If there is not, please feel welcome to upload a case study.
  2. Please obtain consent from the person with diabetes before submitting your case study. Your case study must not include any identifying information or feature of the person living with diabetes. Please ensure you maintain privacy and confidentiality. Do not include name, address, date of birth or any contact details. 
  3. You are not required to identify yourself. However, your email address is collected to provide you with a response. This information is not shared on the case study portal or with moderators.
  4. You will be asked a series of questions in the form below. The questions will appear as you make your way through the online form. Please provide as much detail as you can. You can download a Word version the questions to view and complete them prior to filling out the online form here (link to word doc in resource library).
  5. Once submitted, the case is forwarded to an Expert Advisory Group of CDEs who, will review the information and provide you with broad advice to help you with your clinical assessment and formulate your care plan.  
  6. You can expect to have an answer within 7 working days.
  7. Once finalised, if it is not a replica of other case studies, it will be uploaded into the Case Study Repository under a relevant heading. This provides resources for other diabetes educators struggling with the same challenging cases.
